Job Description:
Job Description Job Description Provider will diagnose and treat
diseases, injuries, and other disorders. They also work to promote
good health and prevent illness. Patient care is to be delivered in
collaboration with and under the direct supervision of the
Physician. Provides quality comprehensive primary health care
services to the Health Center’s patients. This is primarily a
one-provider location with access to the Physician with the
assistance of two nurses and an office manager. Examines,
diagnoses, treats and/or refers patients; prescribes
pharmaceuticals, other medications, and treatment regimens as
appropriate to assessed medical conditions. Maintains a problem
oriented health record for each patient to include providing
appropriate documentation within the Electronic Health Record.
Assures continuity of care by providing comprehensive medical
services on a timely basis as per the Health Center schedule within
the Center’s scope of practice and the provider’s training. Refers
patients for appropriate specialty care services, lab and x-ray and
any other ancillary services that are appropriate for patient’s
management and care. Reviews all lab and x-ray reports for patients
under his/her care on a timely basis and makes medical management
decisions appropriately. Follows established departmental and
clinic policies, procedures, and objectives; continuous quality
improvements objectives and safety, environmental, and/or infection
control standards. Participates in specified health promotion,
education and/or prevention programs as needed i.e. diabetes
collaborative/health fairs etc.; attends and participates in clinic
meetings/departmental meetings and other clinic functions as
required by the Health Center. Education, Knowledge, Skills &
Responsibilities: Successful completion of an approved Physician
Assistant or Nurse Practitioner program. Currently certified and
have an unrestricted license in the State of West Virginia. Must be
registered and have current DEA, and other such certificates to
legally operate as practitioner in the State of West Virginia. Must
be certified by American Heart Association in BLS, and ACLS is
preferred. A minimum of at least 3 year's experience is preferred,
and at least 2 years is required. Participates in other assigned
duties as directed by Chief Medical Officer.
